Our History
Patient’s Choice Home Health Care was created based on the patient’s and family’s point of view of what quality health care should be. We tailor our services according to the specific needs of each client – this makes individualized care more effective, convenient and less costly.
Our company was founded in 2005 by a team of specialists with more than 30 years of health care experience combined.
At Patient’s Choice Home Health Care, we highly emphasize efficient communication between the patient, the clinicians and physicians involved. We continually self-evaluate the quality of care provided and will look for ways to improve. It is our greatest commitment to educate patients on proper self-management of disease processes and ensuring safety at home.
Our Team

We provide the most current home health care training and education to all of our staff members. Proper procedures are always in place for the coordination of patient care to meet specific needs, conditions and settings for our patients.

We make certain, that prior to initial patient care, our staff is cleared by background and license screenings.
